According to the Boone County Sheriff’s Office, the driver of the Humvee was identified as Izaya Phillips, 21, of Ingalls, Indiana. Three passengers were also inside the vehicle: Nicholas Reinhart, 27, of Washington, Indiana; Luke Richardson, 23, of Carmel; and Terry Frye, 23, of Lafayette. According to the Sheriff’s Office, Frye was pronounced deceased at the scene, while the other passengers were transported to a local hospital with injuries.
Deputies said the driver of a white Subaru involved in the crash was identified as Veronica Webster, 51, of Independence, Kentucky. Webster was treated on the scene for her injuries.
The third vehicle, a car hauler truck, was driven by Goderdzi Gujabidze, 56, of Brooklyn, New York. Gujabidze was evaluated at the scene, then transported for further processing due to a language barrier, authorities said.
During the initial investigation, all non-injured drivers were taken to a nearby medical facility for a blood draw and post-accident interviews, consistent with department policy. After Gujabidze’s interview, Boone County deputies were notified by Immigration and Customs Enforcement (ICE) that a hold had been placed on him. The Sheriff’s Office then detained him in accordance with department protocol.
Officials said the cause of the collision remains under investigation. The Sheriff’s Office is working with the National Guard and the Federal Motor Carrier Safety Administration as the review continues.
