According to the Kanawha County Sheriff’s Office, the crash happened at approximately 4:53 p.m. in the 2000 block of Lens Creek Road in the Lens Creek area of Hernshaw, southwest of Charleston.

Investigators said an SUV driven by Kayla Jacobs was involved in a head-on collision with a utility van owned by Mullen’s Plumbing, Heating and Cooling. The van was driven by Jimmy Hager.

The juvenile passenger in the SUV, 8-year-old Ethelyn Grubb, was seated in the front passenger seat and was pronounced deceased at the scene due to injuries sustained in the collision, according to deputies.
